The rank is given within a few minutes. To join a full server after getting Supporter, you have to log in to a non-full server.

Yes. Use the "Gift It" option on any rank card and enter the recipient's SteamID64 or profile URL at checkout. They must have a Steam account linked to receive the rank.
Each rank lists its perks per server on the product card. Shared perks apply everywhere; server-specific perks only apply on that server.
Yes. Purchase a higher tier and PayNow handles the tier change. You only pay the difference for the remainder of your billing cycle when upgrading a subscription.
